TITLE:             Physical Therapist

 

QUALIFICATIONS:    Licensure by the Board of Physical Therapy, Florida Department of Professional Regulations.

 

REPORTS:                   As assigned on current organizational chart.

 

SUPERVISES:

 

POSITION GOALS:     To provide physical therapy to identified students

 

 

PERFORMANCE RESPONSIBILITIES:

 

1.         Provides the Executive Director of School Support Services, Supervisor of School Services, and principals with the pertinent information regarding schedules, changes in schedule and data concerning the therapist’s physical therapy program.

2.                  Conducts inservice training programs for school personnel and parents.

3.                  Screens all students referred by parents, teachers, medical personnel, or outside agencies, and those who come through a screening program.

4.                  Identifies students with motor problems through referral and routine screenings.

5.                  Evaluates students identified through screening as having  noticeable motor deviation.

6.                  Assigns students with a motor problem to the appropriate level of service as determined by criterion set by the district procedures for providing physical therapy services.

7.                  Reevaluates all students periodically assigned to any of the service levels.

8.                  Develops an appropriate therapeutic program to meet the needs of each individual.

9.                  Formulates an individual educational plan with short and long term intervention goals and objectives to meet individual needs.

10.              Attends all staffings, school conferences, and parent conferences as indicated by the need of the child.

11.              Develops and maintain effective working relationships with school personnel and other professionals.

12.              Cooperates with community, county, state and federal programs to effect comprehensive physical therapy services.

13.              Attends inservice meetings which have been planned for professional growth and to explore current information contained in journals, books, and magazines pertinent to physical therapy.

14.              Performs other applicable duties as assigned by immediate supervisor.

 

TERMS OF EMPLOYMENT:           

 

EVALUATION:          Performance evaluated annually in accordance with the Board’s policies on evaluation of licensed personnel.
